The following forms are used to apply to take Principal's Qualification Program courses. All documents are downloadable PDF files which are to be completed in full and returned by email, fax, mail or delivery to:

 

 

It is the student's responsibility to ensure the completed application form and all supporting documents (including payment) are submitted to the Department of Professional Development in Education. If required documentation or payment is missing, your application may not be processed.
